Return All Forms by Aug 1st, 2008 or prior to your registration date.

UD Physical Examination Form must be used.

Per UD and NCAA, the physical examination must be performed after May 31, 2008 or no earlier than six months prior to date of first practice (which ever is first) and completed by a licensed physician, nurse practitioner, or physician's assistant.
