Medical Insurance

 At the beginning of each year, UD Emergency & Medical Information Form must be submitted to the Medical Coordinator office.   As a student, for your health and safety as well as those around you, it is your responsibility to report any medical conditions to the Medical Coordinator's office, if needed I will help you find appropriate medical care.  The forms supply us with valuable information, but not limited to:  medical needs, emergency notification contacts, and insurance information to arrange medical appointments.  Information will remain confidential.  For further information please contact the Medical Coordinator's office.

 Often, students are covered under a policy held by their parents. If this is the case, students should sit down with their parents and go over the coverage with them. Questions to consider are:

• What is covered by the policy?
• What is not covered?
• Is University of Dubuque, Dubuque, Iowa considered "of area" for the policy?
• Is there a deductible?
• Does the student have his/her own insurance card?  Or copy of the insurance card?
• Students should carry their insurance card with them at all times.

  • Will be required for completion of University of Dubuque required forms
  • Will be required in case of medical emergency
