UD Safety Management System

Safety is the #1 priority in the University of Dubuque Aviation Program

Everyone has a major role in promoting safety in the program. We all have the responsibility to keep safety in the forefront of our minds at all times when we are involved in aviation activities. We must be able to acknowledge, evaluate, and improve any hazardous situation that may arise.

The UD Safety Reporting System

The Safety Reporting Form‌ can be found in various locations throughout the Flight Center, in the aircraft binders and online‌. They are to be completed by anyone who witnesses any unsafe event whether it be on the ground or in the air.

As a reporter, you have the option to identify yourself on the Safety Reporting Form. The UD Safety Reporting System is non-punitive. That means we won't exert punishment for submitting a form however, the UD safety staff may contact reporters for more details if necessary.

Aviation Safety Reporting System (ASRS)

Any event that is witnessed and deemed as unsafe may have violated the Federal Aviation Regulations. If you feel that you have witnessed this type of activity, please consult with your flight instructor or the supervisor on duty. You may submit a safety report on the NASA website at http://asrs.arc.nasa.gov/.
