If you have academic-related concerns regarding a student and wish to have University personnel follow up with them, please submit the form below.

Why this matters:

• Reporting the academic concerns allows professional academic support staff to follow up with the students early in the term to help them get back on track.
• Accurate and timely reporting by all instructors lets us determine if a student is having a serious problem across all enrolled courses or if it is an isolated issue.
